Jaw Pain When Chewing — Causes, Assessment, and Management

Jaw pain when chewing is almost always caused by one of three things: a TMJ joint problem (disc displacement or arthritis), a jaw muscle problem (MPDS or bruxism), or a bite problem (uneven tooth contacts from a crown, filling or denture). Each has a different treatment which is why identifying the exact cause matters before starting any therapy.

Jaw Pain When Chewing — Three Main Causes

JOINT ORIGIN (TMJ Disc or Arthritis)

Pain is located in front of the ear. It may be sharp at the start of chewing and ease slightly as the jaw warms up or it may worsen progressively. Clicking or grating sounds are common. Opening wide (big bites) is typically more painful than small movements.

MUSCLE ORIGIN (MPDS / Bruxism)

Pain is felt in the cheek, temple, or side of the jaw not specifically in front of the ear. It builds during chewing and may linger after finishing a meal. Morning jaw stiffness is common. MRI is normal because the joint itself is fine the muscles are the source.

BITE ORIGIN (Occlusal Imbalance)

Pain is triggered specifically when certain teeth come together. It may be worse on one side. It often starts after a new crown, filling, extraction or denture. The pain is usually sharp and localised rather than diffuse. T-Scan bite analysis identifies the problem contact precisely.

How Is Jaw Pain When Chewing Diagnosed?

The key to resolving jaw pain when chewing is differentiating between joint, muscle and bite origins because the treatment for each is different.

At Dr. Prostho Dental Clinic in Mumbai, the evaluation includes:

      • Waking with jaw pain that is worse after meals the day before.
      • Detailed symptom history: when it started, what makes it better or worse, whether there have been recent dental changes
      • Clinical examination: range of movement, joint palpation, muscle trigger point assessment
      • T-Scan digital bite analysis: objective recording of where force concentrates during chewing
      • EMG: muscle activity recording to identify overactive muscles during chewing
      • Selective imaging (MRI or CBCT) when joint pathology needs to be ruled out.

Most cases can be diagnosed clinically without imaging. When MRI is used, it confirms disc position — but a normal MRI does not rule out jaw pain, as muscle-origin pain (MPDS) does not appear on imaging.

Jaw Pain When Chewing — Treatment Options in Mumbai

Treatment is guided by the identified cause:

      • For bite-origin pain: Selective bite adjustment (equilibration) removing the specific high contact typically resolves symptoms within days. This is a precise, conservative procedure guided by T-Scan data, not guesswork.
      • For muscle-origin pain (MPDS): Trigger point management, bite equilibration to reduce muscle loading, ULFTENS therapy, and a stabilisation splint for night-time protection.
      • For joint-origin pain: Anterior repositioning to allow disc recapture (for reducible disc displacement), stabilisation splint for joint protection, and physiotherapy if cervical involvement is present. Surgery is rarely necessary.

Frequently Asked Questions — Jaw Pain When Chewing

Can jaw pain when chewing be cured?

Yes. In most cases. When the correct cause is identified and addressed, jaw pain when chewing resolves or significantly improves with conservative treatment. The key is accurate diagnosis: joint-origin, muscle-origin and bite-origin pain each require a different approach.

Why does chewing hurt on one side of my jaw?

One-sided jaw pain when chewing usually indicates either a unilateral bite problem (one side receiving disproportionate force), MPDS on one side, or unilateral TMJ disc displacement. T-Scan bite analysis is particularly useful for identifying the side with abnormal loading.

Is jaw pain when chewing a sign of TMJ?

It is one of the hallmark symptoms of TMJ disorderr, yes. However, not all jaw pain when chewing is joint-related. Muscle-origin pain (MPDS) and bite imbalance can produce identical symptoms. A specialist evaluation differentiates between these causes.
